2020 MDAM Volunteer Form

Thank you for your interest in volunteering for March Disability Awareness Month preparation!

We need volunteers to fold cardboard mailing boxes; count the number of required materials corresponding to the order and place the materials in a box or envelope; seal mailing boxes and envelopes shut; review the order form to confirm that all required items have been packed; and transport materials using a rolling cart from our office down to the shipping area in the building. You do not need to be able to perform all of these tasks in order to volunteer as we'll only assign you tasks that you're comfortable performing. Accommodations will be provided if requested in advance.
